Our Team

We use a collaborative model of care that recognizes that no single professional discipline is able to meet all the health-related needs of individuals and communities. Although nurse practitioners are our primary providers, they work with a team of physician collaborators, health educators, community outreach workers, nutritionists, counselors, and students/interns to help clients achieve optimum health and well-being.
