This year, CCAHTA will award two $250.00 grants, and one $500.00 grant to members for planning and/or implementation of innovative Horticultural Therapy programs. Applications should be postmarked by April 14, 2000.

Description of proposed project: Include details as applicable regarding type of project, location, time required in preparation, client or audience served, timetable for implementation, benefits or goals, expenses involved and how this grant money will be utilized. Please use reverse side or separate sheet if additional space is needed. Applicants must be current members of CCAHTA. All projects must be non-profit and conceived and implemented by you. Projects must involve plant related activities which improve the quality of life for persons with special needs. Rating will be based upon community need and significant impact, use of collaborative partnerships, and potential as a model program. Funds are not transferable to other project. Recipients will be required to submit a written report at the completion of the project which will be published in the CCAHTA Newsletter. No repayment of funds is required unless recipient fails to complete the project for which the funds are awarded, or fails to make a report of how the funds were put to use, in which case, we would expect a return of the entire grant. Recipient also agrees to allow CCAHTA to submit press releases announcing the grant award to appropriate newspapers.
